BIRTHDAY PARTY BOOKING FORM

while enjoying roller skating fun. Your birthday will be fun…
And easy…and WE’LL CLEAN UP THE MESS!

Please read the following guidelines offered so you can relax and enjoy your party:
1) You may check in 15 minutes before your scheduled party time. Our birthday party server will have your table set up for you! Your server will let you know what time your guest will be called to the party table. The server will light the candles and let the guest sing Happy Birthday. When your party is finished your server will come back and clean your table.
2) No FOOD or DRINKS other than a birthday cake may be brought in from outside the building. Your party includes paper goods, drink and ice cream. Add-ons, such as pizza, hot dogs, pitchers of drinks, etc. may be purchased for your party.
3) Personal items should be placed in the lockers at the end of the skating center for a nominal fee. We are not responsible for lost or stolen items.
4) Regular skate Rental is included in your Birthday Party package. Inline Rental is $3.00 per skater. Most personal skates are allowed. The decision of the manager on duty is final if their personal skates are not acceptable.
5) Upon arriving each guest will receive an armband. The armband will need to be on each guest in order for guest to receive skates from our skate attendant. Put on your skates and enjoy!
6) We will greet any guest who arrives late … and we will give them an armband so they can join you as soon as possible.
7) Please be prepared to be FLEXIBLE on busy birthday party days as your serving time may be delayed 5 – 15 minutes. As soon as we are ready to serve your party we will announce for you and your guest to assemble at the party table. At that time, the fun can begin and you may enjoy refreshments and open your gifts. Sit Back and Relax.... It will be our pleasure to serve you and your guest.
8) Because there are other special birthday party people celebrating their birthdays during the session, PARTY TABLES ARE LIMITED TO 1 1/2 HOURS ONLY! Please be cooperative in completing your party on time. We handle many parties each session and want to be fair to everyone.
9) Please notify your party server as soon as possible regarding extra drinks, pizza, hot dogs, popcorn or anything else you may need.
10) Your Birthday Party Package is not a PRIVATE PARTY unless scheduled as such. You and your guest will be skating with others during the regularly scheduled public session.
11) A $25.00 deposit is required to be paid 7 days before your scheduled party. If you schedule a party with less than 7 days before your party date, a deposit will be required at that time. If your deposit is not received 7 days in advance, you may lose your scheduled party time. Deposits are non-refundable less than seven (7) days prior to your scheduled party. You may bring your deposit in or drop it in the lock box outside if there is not enough time to mail it. If there is any doubt, please drop your deposit off. We do not want you to lose your scheduled party and disappoint your child.
12) Please fill out the bottom of this form and print a copy of the form and drop it off at Skate Zone with your deposit. If you have any question please do not hesitate to give us a call @ 988-1400.
